- Members of the medical scheme are
informed of the HIV/AIDS benefit (or chronic medication benefit) and
HIV/AIDS disease management programme by articles in relevant
newsletters, booklets, brochures, etc. Existing literature routinely
sent to members by the scheme and administrator is used extensively.
- Members who need additional
information, or who want to register themselves onto the HIV/AIDS
disease management programme can phone 012 673 8901.
- A dedicated, experienced HIV/AIDS
disease manager (a professional nurse) will provide information about
the registration procedure and a registration form will be forwarded
to the member.
- Completed registration forms are
returned to Mx Health by using a separate postal address, P.O.
Box 9698, Centurion 0046. Completed forms can also be faxed to 012-
673 8927 (a separate, confidential fax number)
- A HIV/AIDS disease manager will
initiate a sequence of approximately 3 telephone calls over the next
year to give advice and support and to monitor treatment response and
compliance.
- Specific member materials dealing with
information about the infection, various complications, monitoring
blood tests, and lifestyle management are mailed to the participant on
an ongoing basis.
- In specific cases where the medical
scheme is using the medicine management services of Mx Health
Institute for authorisation of chronic medication, the member
needs to forward a chronic medication application form to Mx Health
chronic medication department. If the chronic medication is
HIV/AIDS related, the HIV/AIDS disease manager will call the member
and explain the existence and possibility of enrolment onto the
HIV/AIDS disease management programme. A registration form will be
forwarded to the member and the same process will follow.
